In Lagos, a man with uncontrolled hypertension skips his clinic appointment because the queue starts before dawn and he can’t afford to lose a day’s pay. In Enugu, a nurse handles 80 patients a day because three colleagues left for the UK last quarter.

These are not edge cases. They are everyday realities. And they are the reason why telemedicine in Nigeria is not a convenience story; it is a survival story.

The Problem: A System Under Extreme Pressure
Nigeria’s healthcare system faces a crisis that no single intervention can fix. The numbers tell part of the story:

-    1 doctor per 5,000 people (WHO recommends 1:600)

The figures above represent a system in structural crisis. But numbers alone do not capture the full weight of it.

A Nigerian woman faces a 1-in-22 lifetime risk of dying from pregnancy, compared to 1 in 4,900 in most developed countries.

75% of Nigeria’s healthcare workers are concentrated in urban areas, serving just 45% of the population. Rural communities carry the heaviest burden with the fewest resources.

Brain drain continues to accelerate the problem. Over 12,000 Nigerian doctors are now practising in the UK. The resident doctor workforce has shrunk from 15,000 to approximately 8,000 in the past decade.

The Nigerian Medical Association estimates it would take over 15 years of sustained training without further emigration to close the gap.

This is the context in which telemedicine must be evaluated. Not “is it better than ideal?” but “what is it replacing?” because for millions of Nigerians, the alternative to a virtual doctor is no doctor at all.

**What Is Telemedicine?**

In simple terms, speaking to a doctor remotely via phone, video, or app without going to a hospital.

It includes virtual consultations, remote monitoring of chronic conditions, digital prescriptions, mental health support, and pharmacy delivery.

Nigeria’s digital health market was valued at US$770 million in 2024, with over 12 active telemedicine startups and more than 5,000 Nigerians employed in the sector.

This is not a future story. It is already happening.

Where Telemedicine Is Making a Real Difference

1.  Cancer Care - Reaching Patients Who Had No Other Option
    
Nigeria records over 269,000 new cancer cases annually, but has only 27 cancer treatment centres for a population of over 200 million.

For many patients, travelling to Lagos or Abuja is simply not an option.

Teleoncology has been shown to reduce patient costs by up to 10%, eliminating travel and accommodation expenses in a system where over 90% of healthcare is paid out-of-pocket.

1.  Chronic Disease Management - Consistency Over Distance
    
Diabetes, hypertension, and cardiovascular disease account for nearly one-third of deaths in Nigeria. These conditions require consistency - not just access.

Telemedicine is already expanding access, reaching over 30% of patients with hypertension and diabetes through remote care.

A 2024 study published in a BioMed Central (BMC) journal supports this, piloting mobile hypertension training for primary healthcare workers in Nigeria and improving both patient access and frontline healthcare capacity.

Virtual prenatal care pilots in Nigeria have also been associated with a reported 20% reduction in maternal mortality in some trial areas.

1.  Mental Health - Breaking Through Stigma and Distance
    
Over 80% of Nigerians with a diagnosable mental health condition receive no care at all. Nigeria has approximately one psychiatrist per one million people.

Telemedicine doesn’t eliminate stigma - but it makes care accessible despite it.

A person who would never enter a psychiatric ward may still open an app from the privacy of their home.

Organisations like Mentally Aware Nigeria Initiative (MANI) have successfully pivoted to digital platforms - using WhatsApp groups, hotlines, and virtual communities to reach users.

Over 7 million Nigerians have been reached through virtual mental health platforms.

**What Telemedicine Cannot Do**

Honesty matters here.

Telemedicine cannot replace emergency care, physical examinations, surgeries, procedures, laboratory tests, imaging, or diagnostics.

And in Nigeria’s context, practical barriers remain:

-    Only around 50% of Nigerians have reliable internet access
    
-    Over 90% of rural communities lack stable electricity
    
-    Low digital literacy limits adoption in older populations
    
-    Trust gaps and regulatory uncertainty around data privacy remain
    
-    The 2025 health budget allocates just 5% to digital health
    

A 2025 review of 230 digital health studies in Nigeria found that while research and innovation are growing, real-world implementation remains limited.

Enthusiasm does not automatically translate into impact.

**The Government’s Role: Why Policy Cannot Wait**

Without policy action, telemedicine risks becoming a solution for the few - not the many.

To scale impact, key actions are required:

-    Increase healthcare funding toward the WHO-recommended 15%
    
-    Integrate telemedicine into the National Health Insurance Scheme
    
-    Invest in broadband and electricity infrastructure
    
-    Establish clear regulatory frameworks for data privacy and clinical standards
    
-    Strengthen public-private partnerships
    
**Conclusion: A Necessary Bridge, Not a Silver Bullet**

Telemedicine will not replace the specialist that the woman in Kebbi needs.  
It will not stop a doctor from boarding a flight to London.  
It will not build the hospital that should already exist.

But it can reach her before a crisis begins.  
It can give the man in Lagos access to care without losing a day’s income.  
It can extend the reach of a healthcare system already under strain.

The real opportunity is not telemedicine versus physical healthcare - it is both, working together.

Because in Nigeria today, telemedicine is not replacing care. It is making access possible.

Local infrastructure is frequently **compromised** by:

-    Hardware failure across switches, routers and cabling

We’ve seen servers shut down because the air conditioning in server rooms failed.  
We’ve seen network equipment damaged during lightning strikes in poorly earthed buildings.

**The Hidden Risk in Fully Offline Systems**

Hospitals concerned about cybersecurity often default to fully isolated (“air-gapped”) local servers — sometimes even rejecting VPN access.

A couple of weeks ago, I had an interesting conversation with an EHR founder. Their product is live, with paying customers and active deployments.

But they admitted something uncomfortable: they don’t believe their database is usable for AI workflows. In their words, the data isn’t clean enough. The structure isn’t reliable enough. Building intelligence on top of it would mean months of painful cleanup (if it’s even possible).

Their proposed solution? Rewrite the system on an openEHR base so the underlying PostgreSQL structure becomes more AI-friendly (I’m still not sure how I feel about that approach).

But the conversation surfaced something much bigger than openEHR vs non-openEHR. It revealed a gap that most hospital owners don’t see coming.

**The Intelligence Readiness Gap**

Many EHRs are built to digitize paper. Very few are built to power intelligence. That difference matters.
A system can generate bills, print prescriptions, store notes and track admissions, but still be structurally unfit for analytics. That system is not necessarily a strategic asset: it may simply be paper… in a computer. Documentation is not intelligence.

Why?

Because documentation and computability are not the same thing.

If diagnoses are mostly free text…  
If lab results are stored as PDFs…  
If data fields are inconsistently used…  
If there’s no enforced coding structure…  
If longitudinal queries are hard or impossible…

Then what you have is a documentation layer — not a data engine.
And AI does not thrive on documentation. It thrives on structure.

Put another way: if your system cannot answer those questions without exporting spreadsheets and manually cleaning data for weeks, you don’t have operational intelligence. You have a record-keeping tool. And record-keeping tools do not grow topline.

Anyone who follows healthcare AI founders globally has seen this pattern. Teams trying to build AI tools on top of existing EHRs often spend months just cleaning chaotic data before they can even begin model development.

That’s not because AI is immature or because the team lacks talent. It’s because the underlying systems were never built for computability in the first place.

The EHR did its original job: it digitized workflows. But it was never designed as an intelligence substrate.

**Why Would an EHR Company Do This?**

Three honest reasons.
1. Speed beats structure.

Early-stage companies optimize for survival. Go live fast. Close contracts. Adapt to client requests. Deep schema design slows you down.

2. Hospitals rarely demand structured data.

During procurement, hospital owners ask:
-    Can I bill from it?
-    Can I generate reports?
-    Is it affordable?
-    Will my staff adopt it?
    
They almost never ask:
-    Are diagnoses coded?
-    Can I run cohort-level queries?
-    Is this system intelligence-ready?
    
Vendors respond to buyer incentives.

3. Analytics wasn’t the original product.

Most African EHRs began as digitization tools, not operational intelligence systems. That origin story shapes architecture decisions in ways that are hard to reverse later.

**Why This Should Worry Hospital Owners**

Because the next phase of hospital competitiveness won’t be about “having software.” It will be about visibility.
Visibility into:
-    Which hypertensive patients haven’t returned in 6 months
-    Which antenatal patients are at risk of dropping off
-    Which families haven’t re-engaged
-    Which service lines are quietly underperforming
-    Where revenue leakage is occurring
    
If your EHR cannot answer structured questions about your own patient base without heroic manual effort, you are not intelligence-ready.
And that becomes a strategic liability, especially if you’re locked into a multi-year contract.

**The Contracting Phase Blind Spot**

Most small and mid-sized hospitals in Nigeria do not have a CIO. They do not have internal data architects, nor do they evaluate schema design during procurement.
So how would they even know?

A few simple questions expose the difference immediately:
-    Are diagnoses stored as structured codes or free text?
-    Are lab results stored as discrete values or uploaded documents?
-    Can I query “all diabetic patients with no visit in 6 months” in real time?
-    Do you provide structured API access?
-    Can you demonstrate cohort-level analytics from a live deployment?
    
If a vendor cannot show cohort queries on live data, that’s a signal. Because intelligence cannot be bolted on easily later.

**This Is Not About openEHR**

Standards matter. Architecture matters.
But this isn’t primarily about whether a system uses openEHR.
You can build a chaotic implementation on openEHR. You can build a disciplined, intelligence-ready system without it.

The real question is simpler:
Was this EHR designed as a documentation tool — or as a computable clinical data engine?
That design philosophy determines whether AI becomes a layer you can activate… or a rebuild you must fund.

**The Bigger Strategic Question**

In the coming decade, hospitals won’t compete on who has the most features. They will compete on who can see patterns inside their own data.

Revenue visibility.  
Recall systems.  
Preventive enrollment.  
Leakage detection.  
Service line optimisation.

All of that requires structured, queryable, longitudinal data. If your system cannot support that, you don’t just have a tech limitation: you have a strategic ceiling.

And ceilings are expensive to break through later.

The headlines tend to focus on “AI will fix healthcare.”  The real question is much harder:

**Which specific, agentic AI use cases meaningfully solve our most consequential problems — for the actual healthcare workers on the ground?**

Community health officers in PHCs.  
Midwives managing antenatal care and labour.  
Nurses.  
Doctors.  
Pharmacists.  
Laboratory professionals synthesising fragmented patient data.

Where can AI safely augment workflows?  
Where can parts of the job genuinely be handed off?

Some use cases are obvious — and still underexplored:
- Medical scribing
- Intelligent billing
- Chronic disease follow-up and monitoring
- Coordinating care for patients with multimorbidity across multiple facilities

But then comes the commercial reality.

Using $200/month AI subscriptions as a benchmark:

Why would a small private hospital owner in Ibadan pay for AI billing automation when they can hire a human for ₦50,000/month?

Why pay for AI-generated clinical notes when junior doctors are inexpensive (read “grossly undervalued”)?

If the promise is efficiency or upside, can that upside be demonstrated before the cheque is written?

Healthcare executives are solving for two questions:
1. Does this increase revenue?
2. Does this reduce cost?

In a low-labour-cost environment, the second question is less compelling than Silicon Valley assumes.
Then layer in infrastructure realities:
- Limited in-country data centre capacity
- GPU constraints
- Power and internet reliability
- Regulatory preferences for in-country hosting
- Cost implications of Azure/AWS/GCP GPU rentals
- Model selection trade-offs (proprietary vs open-source, geopolitical considerations)

This is not a hackathon problem.
So the real question for any serious African healthtech founder building in AI is:

**Is my problem space clinically consequential — and can I demonstrate commercially relevant pilot data that scales affordably within our infrastructure and policy constraints?**

Not activity.  
Not noise.  
Results.

Beyond the AI Hype: From AI promise to practical healthcare impact in Nigeria

There’s a version of the AI conversation in healthcare that feels imported.

Clinical copilots.  
Autonomous diagnosis.  
AI-driven billing and coding.  
Large language models “transforming medicine.”

Important? Absolutely.  
Relevant long-term? Of course.

But if you’re running a private hospital in Nigeria today, that’s not where you start.

We’re deeply excited about AI in drug discovery: de novo molecule design, toxicity prediction, genomics. That’s frontier work. But if I owned a 30-bed hospital in Lagos right now, my first AI question wouldn’t be:

“How do I automate clinical reasoning?”

It would be:

**“What revenue opportunities am I missing, and how do I see it earlier?”**

Because let’s be honest. In our environment, growth funds innovation. Not the other way around.

My second question would be:

**“Where am I quietly losing money - and how do I plug the leaks?”**

That order matters. If I can grow topline by 50%, I’ll worry about shaving 5–10% off costs later. Especially in a cash-constrained setting where margins are thin, marginal costs are low, and capital is expensive.

**The uncomfortable truth**

Most private hospitals under-monetise their existing patient base.

Patients come once. They don’t return.  

Follow-up is inconsistent.  

Screening is opportunistic.  

Preventive programs are informal.

Not because owners don’t care, but because there is no structured intelligence layer asking the right questions consistently.

Questions like:
-    Which hypertensive patients haven’t returned in 6 months?
    
-    Which antenatal patients missed scheduled visits? Can an AI agent send personalised reminders and prompt the front desk to rebook them?
    
-    Which diagnostic tests should a junior doctor be requesting with a particular range of presenting complaints, that we’re legitimately under-screening through an insufficient history or inexperience?
    
-    Which high-value families haven’t re-engaged?
    
-    Which patients qualify for structured preventive packages but were never enrolled?
    
This isn’t “growth hacking.”

It’s disciplined recall.  

It’s operational intelligence.  

It’s revenue hiding in plain sight.

And here’s the key point: AI doesn’t replace your staff here. It augments attention.

It surfaces:

-    Who to call
    
-    Who to nudge
    
-    Who to invite
    
-    Who to enrol
    
In many Nigerian hospitals, the owner is seeing patients, performing procedures, managing staff, negotiating with suppliers, and somehow also “running” the hospital. Administration becomes the side hustle. Intelligence falls through the cracks.
AI, used properly, becomes the missing layer between clinical activity and business performance.

**What can be done directly inside the hospital?**

Plenty.

-    Structured recall systems for chronic disease
    
-    Automated antenatal retention workflows
    
-    Screening prompts embedded into consultations
    
-    Family-based engagement tracking
    
-    Preventive care package identification
    
-    Revenue cycle alerts before claims leak

    
None of this requires futuristic medicine. It requires visibility.

In a cash-constrained healthcare environment, revenue expansion beats cost-cutting almost every time.

That principle sits at the core of how we think about hospital-focused AI at Plural: not as a clinical gimmick, but as an operational intelligence engine that protects margins and uncovers growth.

**The harder question**

What revenue-relevant, patient-facing workflows could be safely delegated to AI — with appropriate guardrails?
